Annual In-Person Compliance Requirement

Beginning January 1, 2025, patients prescribed controlled medications must complete an in-person visit once every 12 months. This visit can be scheduled directly with our team or fulfilled by your primary care provider using our referral form.

Two Ways to Complete Your Annual In-Person Visit

All patients prescribed controlled substances must complete an in-person visit once every 12 months. You can meet this requirement in either of the following ways:

Have Your PCP Complete the Collaborative Practitioner Form

Your Primary Care Provider can complete a wellness visit and submit our collaborative practitioner form. Print the form, bring it to your PCP for signature, and have your PCP's office fax the completed form directly to us at (833) 941-5047. This ensures proper documentation and compliance.

Federal & DEA Requirements

This policy follows DEA guidance and the Ryan Haight Act, which require an in-person evaluation to continue prescribing controlled substances:

  • Once within the first 6 months (if applicable)
  • At least annually thereafter
  • To ensure safe and compliant prescribing practices

Flexible Options for Completion

This visit can be completed with your provider at Mile High Psychiatry or with your Primary Care Provider. If using the PCP option, patients must print our collaborative practitioner form, bring it to their PCP for signature during an in-person visit, and have the PCP's office fax the completed form directly to us at (833) 941-5047 to ensure proper documentation and compliance.

Using the Collaborative Practitioner Referral Form

If your Primary Care Provider (PCP) is completing the required in-person visit, you'll need to print our form and bring it to your appointment. Your PCP will then complete and fax the form directly to us.

Instructions for Patients

  2. Bring the printed form to your in-person appointment with your Primary Care Provider.
  3. Ensure your PCP completes the form during your visit and has their office fax it directly to us.

Instructions for Your PCP

  1. Perform an in-person evaluation that includes a behavioral or psychiatric assessment (e.g., mood, substance use, sleep, medication response).
  2. Include this assessment in the visit documentation as part of the patient's care plan.
  3. Complete the referral form and have your office fax it directly to Mile High Psychiatry at (833) 941-5047.
